No period after a chemical pregnancy

Ovulation can happen before the next period, so pregnancy is possible if you have had sex without contraception. The timing of bleeding varies, and the date of the loss may not become the first day of a predictable cycle.

A chemical pregnancy is a very early pregnancy loss detected by a pregnancy test. Hormone levels usually decline afterward, and the ovaries then resume their cycle. Stress, illness, weight change and existing ovulation difficulties can also affect when menstruation returns.

Persistent absent periods can occur with conditions that disrupt ovulation, including polycystic ovary syndrome or premature ovarian insufficiency. These conditions have other clinical features and require assessment; an absent period after one early loss does not establish either diagnosis.

Take a home pregnancy test if your period is late and pregnancy is possible. If it remains positive after the loss, or turns positive again, contact a clinician for advice because repeat testing or an ultrasound may be needed to clarify what is happening.

Track bleeding, pain, test results and dates. Arrange a routine appointment if periods do not resume, cycles remain markedly different from usual, or you want guidance on trying again or contraception. A clinician may review medicines, cycle history and symptoms before deciding whether blood tests or imaging are useful.

Seek urgent medical assessment for severe or one-sided abdominal pain, shoulder-tip pain, fainting, marked dizziness, very heavy bleeding, fever or feeling acutely unwell. These symptoms require evaluation even if bleeding was assumed to be a chemical pregnancy.

A positive pregnancy test with pain or bleeding needs timely review because the pregnancy location may not yet have been confirmed. Emergency care is appropriate if pain is intense, you collapse or you feel weak and clammy.

Ask whether you should repeat a pregnancy test, whether the previous pregnancy location was confirmed, and when an absent period needs investigation in your situation. Mention breast milk production, hot flushes, acne, excess facial hair or major weight changes because these can guide the assessment.
