The short answer
Pregnancy-related fluid, intravenous fluids, reduced movement and hormonal changes can leave both feet and ankles puffy after birth. Swelling that is unequal, tender, red or warm can signal a clot and should not be watched at home.
What the pattern can show
Uncomplicated fluid swelling is often similar on both sides and may feel worse after standing. It should trend down as the body removes extra fluid. Pressing the skin may leave a temporary dent.
A clot in a deep leg vein may present with swelling mainly in one leg, calf or thigh pain, warmth, redness or a heavy feeling. Postpartum high blood pressure problems can present with swelling alongside a severe headache, vision changes or pain high in the abdomen.
Ways to manage mild swelling
Rest with the legs supported, change position regularly and take gentle walks if your maternity team has said movement is safe. Wear loose clothing, drink normally and follow the wound-care and activity advice given after surgery.
Compare both legs and note whether swelling is improving. Avoid forceful calf massage when a clot is possible. Do not start water tablets, compression garments or herbal remedies unless a clinician familiar with your postpartum recovery recommends them.
Signs that need urgent care
Seek urgent same-day assessment for new swelling in one leg, calf or thigh pain, redness, warmth, rapidly increasing swelling, or difficulty walking because of pain. These features require examination for a possible blood clot.
Call emergency services for sudden breathlessness, chest pain, coughing blood, collapse or severe weakness. Urgent maternity assessment is also needed for a severe headache, visual disturbance, upper abdominal pain, vomiting, or feeling acutely unwell after birth.

Explain when the swelling began, whether it affects one or both legs, and whether pain, redness, headache or breathing symptoms are present. Ask what activity is suitable after surgery, whether your blood pressure needs checking, and whether your clot risk or medicines change the plan.

Is swelling in both feet expected after a C-section?
Mild swelling in both feet or ankles can follow pregnancy, surgery and intravenous fluids. It should gradually improve. Contact your maternity team if it worsens, persists or occurs with other symptoms.
Should I massage a swollen calf?
Do not massage a calf that is newly swollen, painful, warm or red because a blood clot needs to be ruled out. Seek urgent medical assessment instead.
Can I use compression socks after surgery?
Use them only if your maternity or surgical team confirms the correct type and fit. Compression is not a substitute for assessment when swelling is one-sided or painful.
