Can you have a vaginal birth after a caesarean?

This is often called vaginal birth after caesarean, or VBAC. A planned repeat caesarean is the main alternative. Neither route is automatically right for everyone, so review your previous operation details and current pregnancy with an obstetric clinician.

The team considers the type of incision made in the uterus, why the earlier caesarean was needed, any previous vaginal births, the number of uterine operations, placental position, the baby's position and your wider health. The skin scar does not reliably show the uterine incision type, so an operation note can be useful.

Labour places pressure on the uterine scar. For that reason, VBAC is planned in a unit able to monitor labour and respond quickly if concerns arise. Induction may change the options and requires an individual discussion about methods and monitoring.

Book antenatal care and tell the team about every previous caesarean or uterine operation. Bring the operative record if it is available. Discuss your priorities, pain-relief choices, the recommended place of birth, what happens if labour starts before a booked caesarean, and circumstances that would prompt a change of plan.

Ask when to contact the maternity unit once contractions or waters breaking begin. A flexible plan is valuable because the baby's position, placental findings or your health may alter the recommendation later in pregnancy.

Follow your maternity unit's instructions when labour begins. Seek urgent assessment for heavy vaginal bleeding, severe constant abdominal pain, collapse, breathing difficulty, reduced or changed fetal movements, or any sudden concern about your wellbeing or the baby's wellbeing. During labour, continuous or close monitoring may identify changes that require prompt review.

Ask what your previous operation record says about the uterine incision, whether this pregnancy has factors that affect VBAC, how labour will be monitored, and which induction methods could be considered. Clarify the backup plan if labour does not progress or signs of scar difficulty appear.
