Advanced melanoma stages: what they mean

Regional disease may involve nearby lymph nodes, skin or lymphatic channels. Distant metastatic disease may involve organs such as the lungs, liver, brain, bones or distant skin. People with the same broad stage can still have different sites, tumour features and treatment plans.

Staging combines findings from the original melanoma, lymph-node assessment, imaging and sometimes biopsy of a suspected spread site. The team also considers whether disease can be removed surgically, whether it is causing symptoms and which molecular features may open specific treatment choices.

A stage can be refined when new imaging or pathology becomes available. This does not mean earlier care was wrong; it means the cancer's extent is now described with more information. Ask for the sites of disease to be shown on the scan and explained in plain language.

Care is usually coordinated by a multidisciplinary cancer team. Immunotherapy, targeted medicines for eligible tumours, surgery and focused radiation may be used in different sequences or combinations. Treatment may aim to control disease throughout the body, remove selected deposits, relieve symptoms or reduce the chance of further spread after surgery.

Keep an updated medicine list, record new symptoms and attend planned blood tests, scans and skin examinations. Before starting therapy, ask which immune, neurological, breathing or digestive symptoms require a same-day call because some treatment reactions need early management.

Contact the cancer team urgently for a new severe headache, seizure, confusion, weakness on one side, loss of balance, sudden vision change or repeated vomiting. These symptoms can have several causes, but possible pressure or involvement in the brain needs prompt assessment.

Urgent evaluation is also needed for new breathlessness, chest pain, coughing blood, uncontrolled pain, black or bloody stools, jaundice, inability to keep fluids down or rapid deterioration. If symptoms are severe or the oncology team cannot be reached, use emergency services.

Ask where melanoma has been found, which tests confirmed it, whether molecular testing affects options, what the goal of the proposed treatment is, and how response will be checked. Clarify whom to call after hours, which side effects are urgent, whether surgery has a role and how symptoms or emotional strain will be supported.
