Advanced male pattern baldness: options and next steps

Medicines can help suitable patients protect or strengthen miniaturising hair, but they do not reliably rebuild every bare area. Surgery redistributes existing follicles and therefore requires careful prioritisation of the hairline, front, mid-scalp and crown.

Male pattern hair loss develops when genetically susceptible follicles respond to androgens by producing progressively finer, shorter hairs. It commonly presents with a receding front and thinning over the crown while the back and sides are relatively preserved.

With extensive loss, fewer miniaturising follicles remain available to thicken and a larger area needs cosmetic coverage. The permanent donor zone is not unlimited, and the crown can consume many grafts, so a dense-looking result across the entire scalp may not be feasible.

Start with a scalp examination to confirm the pattern and look for inflammation, scarring or diffuse shedding. Discuss prescription treatment, topical options, cosmetic fibres, scalp micropigmentation, hair systems and transplantation according to your health, preferences and remaining hair.

If considering surgery, ask for a donor-density assessment and a design that anticipates future thinning. Medicines require an individual discussion of contraindications, side effects and monitoring. Take consistent photographs to track change rather than relying on day-to-day mirror impressions.

Seek a dermatology review if loss is sudden, occurs in sharply defined patches, affects the eyebrows or body hair, or comes with scalp pain, marked itch, scale, pustules or scarring. These features are not typical of uncomplicated male pattern loss and may signal another process.

Arrange timely care if shedding follows a new medicine or significant illness, or if hair loss is affecting mood, sleep or daily life. Before any transplant, active inflammation or unexplained shedding should be diagnosed and controlled.

Ask how the diagnosis was confirmed, whether follicles in the thin areas are still viable, what each option can achieve, which side effects apply to you, and how progression will be monitored. For transplantation, clarify donor limits, coverage priorities and what happens if native hair continues to recede.
