Advanced heart failure and low blood pressure

A low reading is most concerning when it is new or accompanied by dizziness, fainting, confusion, cold or clammy skin, reduced urine, chest discomfort or worsening breathlessness. Treatment aims to protect blood flow to vital organs while preserving medicines that support the heart whenever clinically possible.

In advanced heart failure, the heart may not pump enough blood forward to maintain pressure. Diuretics can reduce congestion but may also contribute to dehydration; other heart medicines deliberately lower pressure as part of their effect. Infection, bleeding, an abnormal rhythm, kidney problems and poor fluid intake are other possibilities.

Some people live with a lower reading without symptoms, so clinicians assess the change from the person's usual level as well as alertness, circulation, breathing, fluid balance and kidney function. Dizziness can accompany low pressure, but it may also have another cause.

Sit or lie down if light-headed to reduce the chance of falling. Recheck the pressure after resting if your care plan includes home monitoring, and write down the reading, pulse, symptoms, recent weight changes and when medicines were taken. Contact the heart-failure team promptly for a new symptomatic drop or repeated readings below the threshold they provided.

Do not independently stop heart medicines, take extra salt, increase fluids or skip a diuretic. Those actions can worsen congestion or destabilise treatment. Your team may examine you, review doses and timing, check blood tests, assess rhythm, or look for infection and bleeding before deciding what to adjust.

Call emergency services for fainting, severe or rapidly worsening breathlessness, chest pain, confusion, blue or grey lips, cold clammy skin, or inability to stay awake. Very little urine together with weakness or breathlessness also needs urgent assessment. These signs may indicate inadequate circulation or acute worsening of heart failure.

Seek same-day advice for new dizziness on standing, repeated near-fainting, vomiting or diarrhoea that limits fluids, a fast or irregular pulse, or a sudden change in swelling or weight. Have another person stay nearby if you feel unsteady.

Ask what blood-pressure range is acceptable for you, which symptoms matter more than the reading, and whom to contact outside clinic hours. Request a written plan for illness with vomiting, diarrhoea or fever, including how medicines and fluid limits should be handled. It is also useful to discuss fall prevention and the goals of treatment if low pressure repeatedly limits heart-failure therapy.
