Adrenal metastases from lung cancer

An adrenal spot on a scan does not by itself prove metastasis. Clinicians assess its appearance, compare it with earlier imaging and decide whether further imaging or a tissue sample would change management.

A confirmed adrenal metastasis shows that lung-cancer cells have travelled beyond the lung. The team also checks whether cancer is present elsewhere because the number and location of disease sites shape the treatment goal and choice of therapy.

Many adrenal metastases cause no specific symptoms. A larger deposit can produce side or upper-abdominal discomfort. Significant loss of adrenal hormone function is more concerning when both glands are extensively affected, and can cause profound weakness, nausea, low blood pressure, dizziness or confusion.

Your oncology team will consider the lung-cancer subtype, molecular features, previous treatment, other areas of disease, symptoms and overall health. Genomic tumour profiling may help identify a suitable medicine for some tumours. Systemic therapy treats cancer cells throughout the body.

When disease is limited to a small number of sites, local treatment such as radiotherapy or an operation may be discussed for selected people. Palliative care can address pain, appetite, fatigue, breathing concerns and family support while other treatments continue.

Contact the cancer team urgently for severe or rapidly increasing side pain, repeated vomiting, inability to keep fluids down, marked dizziness, new confusion or unusual profound weakness. Emergency help is appropriate for fainting, collapse, severe breathlessness or coughing up blood. These symptoms need assessment and should not wait for a routine appointment.

Ask how certain the team is that the adrenal lesion is metastatic lung cancer, whether another test would alter care, and whether disease is present at other sites. Clarify whether treatment is systemic, directed at the adrenal lesion or both, and how the team will judge response.
