Fatigue can follow poor sleep, intense demands, infection, dehydration, low iron, medication effects, or alcohol use. It can also accompany conditions involving the blood, gut, kidneys, hormones, or nervous system. The symptom alone cannot identify one cause.

How clinicians approach exhaustion

A useful assessment separates sleepiness, muscle weakness, breathlessness, low motivation, and reduced stamina because people may call all of them exhaustion. Timing also matters: sudden or gradual onset, whether rest restores energy, and whether activity causes a delayed worsening can point the evaluation in different directions.

True adrenal insufficiency can present with marked weakness and may include weight loss, vomiting, abdominal symptoms, dizziness, low blood pressure, salt craving, or darker skin. Those features require medical assessment; they should not be diagnosed through a wellness questionnaire or an unvalidated home test.

Prepare for an assessment

Book an appointment when fatigue persists, returns repeatedly, or limits work, study, exercise, or self-care. Bring a list of prescriptions, supplements, recent illnesses, diet changes, sleep quality, and other symptoms. Mention bleeding, snoring, fever, pain, bowel changes, thirst, or changes in urination.

Until reviewed, favour regular fluids and balanced meals, keep a stable sleep schedule, and pace demanding activity. Do not start adrenal hormones, glandular extracts, or stimulant mixtures on your own. Prescribed corticosteroids must be adjusted only with clinical guidance.

Warning signs

Obtain urgent care for fainting, confusion, inability to stay awake, severe or increasing weakness, repeated vomiting, very little urine, or signs of substantial dehydration. Chest pain, severe breathlessness, new one-sided weakness, or suicidal thoughts warrant emergency help.

Prompt assessment is also appropriate when fatigue occurs with black stools, unusual bleeding, persistent fever, unexplained weight loss, or a rapid decline in daily functioning.
