Adrenal carcinoma and lung cancer: what the wording may mean

The distinction affects staging and treatment. Doctors interpret the scan pattern, compare any tissue sample with the lung tumour, and consider molecular test results rather than relying on the wording alone.

An adrenal mass found during lung-cancer assessment is not automatically cancer. It may be a non-cancerous adrenal growth, lung cancer that has spread, or an unrelated adrenal-cortex cancer. Imaging can describe its size, structure and activity, but a multidisciplinary team may need further testing to establish its origin.

When cells from a lung tumour spread to an adrenal gland, the disease is still named and treated as lung cancer. A tumour arising from adrenal-cortex cells is a different cancer, even when a lung abnormality is also present.

Ask which finding is confirmed and which remains uncertain. The team may review chest and abdominal imaging, hormone tests, prior scans, biopsy findings and genomic tumour profiling. A biopsy is considered selectively because its usefulness and safety depend on the mass, suspected diagnosis and whether the result would change treatment.

Treatment is tailored to the cancer's origin and extent. Options for lung cancer can include systemic medicines, radiotherapy or surgery in selected circumstances. Symptom control and palliative care can be provided alongside cancer-directed treatment at any stage.

Contact your clinical team promptly for new severe weakness, persistent vomiting, fainting, confusion, rapidly worsening breathlessness, coughing up blood or pain that cannot be controlled. Call emergency services for collapse, severe breathing difficulty or sudden confusion. These features need urgent assessment because several cancer-related or hormonal problems can cause them.

You could ask: Does the adrenal finding look like spread from the lung, a primary adrenal tumour or a benign growth? Would hormone testing, biopsy or genomic testing change the plan? What is the treatment goal, which side effects need an urgent call, and who should I contact between appointments?
