Adnexal hydrosalpinx: what the finding means

It is often identified on pelvic imaging. Management depends on symptoms, whether the diagnosis is certain, whether one or both tubes are involved, and whether pregnancy is a current goal.

The fallopian tube normally provides a passage between the ovary and uterus. When its far end is sealed by scarring or damage, secretions can collect and stretch the tube. On a scan, this may appear as an elongated fluid-filled structure in the adnexa, the region beside the uterus.

Previous pelvic infection is one possible cause; prior pelvic surgery, endometriosis or earlier inflammation may also be relevant. Some people have no symptoms. Others develop ongoing or intermittent pelvic discomfort, unusual discharge, or difficulty becoming pregnant.

A scan report can suggest hydrosalpinx but must be interpreted with the clinical history because other fluid-filled pelvic structures can look similar. Ultrasound is commonly used, and a fertility specialist may recommend further testing when tubal function will change the treatment plan.

Book a gynaecology review and bring the written scan report and any earlier imaging. Be ready to discuss previous pelvic infections, surgery, pregnancies, fertility treatment, pain and discharge. Testing for infection may be appropriate when symptoms or history suggest it.

Observation may suit an incidental finding that is not causing symptoms and is not affecting a pregnancy plan. When pain, recurrent infection concerns or fertility treatment make the tube clinically important, options can include surgery to remove or separate the affected tube from the uterus.

A hydrosalpinx can interfere with natural conception because the affected tube is blocked. For people planning assisted reproduction, the specialist may discuss treating the tube before IVF. The decision should account for the other tube, ovarian reserve, surgical history and personal priorities.

Seek urgent care for sudden or severe pelvic pain, especially with vomiting, faintness, shoulder-tip pain, fever or heavy bleeding. If pregnancy is possible, pain or bleeding requires prompt assessment to exclude an ectopic pregnancy, which cannot be diagnosed from symptoms alone.

Contact a clinician promptly for fever with pelvic pain, foul-smelling discharge or feeling markedly unwell, as these features can accompany pelvic infection. A stable scan finding without these warning signs can usually be addressed through a scheduled specialist appointment.

Ask how confident the scan diagnosis is, which side is affected, whether the other tube and ovaries look healthy, and whether more imaging or infection testing is needed. If pregnancy matters to you, ask how observation, salpingectomy and IVF would fit your individual plan.
