ADHD medication and high blood pressure

Having high blood pressure does not automatically rule out ADHD treatment. The choice depends on the medicine, blood pressure control, cardiovascular history, other medicines and the balance of expected benefit and possible harm.

Stimulant medicines increase signalling by chemicals that can also affect the heart and blood vessels. Certain non-stimulant medicines may raise blood pressure or pulse, while others can lower them. Individual responses vary, which is why a reading taken only once cannot describe the whole pattern.

A prescriber will consider hypertension, chest symptoms, fainting, palpitations, kidney disease, sleep apnoea and previous heart or circulation problems. Decongestants, caffeine, nicotine, supplements and some prescribed medicines may also influence blood pressure or pulse and should be disclosed.

Take the medicine exactly as prescribed and do not change or stop it solely because of one home reading. Contact the prescriber for advice. If home monitoring is recommended, use a validated upper-arm device, sit quietly beforehand, support the arm at heart level and record readings with the time, symptoms and medication timing.

Bring the log and a complete medicine list to follow-up visits. The clinician may repeat measurements, address sleep and other cardiovascular risk factors, adjust the dose or timing, treat hypertension, or consider another ADHD medicine. The right response depends on the repeated pattern and clinical context.

Seek urgent medical assessment for chest pressure or pain, severe breathlessness, fainting, a sudden severe headache, new weakness or numbness on one side, facial droop, trouble speaking, confusion or sudden vision change. These symptoms matter regardless of the blood pressure reading. Use local emergency services when symptoms are severe or begin suddenly.

Ask how often blood pressure and pulse should be checked, what change should trigger contact, whether other medicines or caffeine could interfere, and which ADHD alternatives fit your cardiovascular history. Confirm who will manage hypertension and how information will be shared between clinicians.
