The short answer
The pattern can include losing track of instructions, starting tasks without finishing, interrupting, restlessness, forgetfulness or strong emotional reactions. Its form varies by person, age, setting, sleep and the demands of the task.
What these behaviours can mean
Someone with ADHD may focus deeply on an engaging activity yet struggle to direct attention toward routine work. Impulsive speech or movement can happen before there is time to pause and consider the situation. Difficulties with planning, working memory and shifting between activities may look like carelessness or refusal from the outside.
Behaviour alone does not confirm ADHD. Sleep problems, anxiety, learning difficulties, mood changes, hearing or vision problems and stressful circumstances can produce overlapping signs. Assessment considers whether a persistent pattern affects daily life in more than one context.
Ways to respond constructively
Use brief, specific instructions and ask the person to repeat the next action in their own words. Break longer work into manageable parts, make routines visible, reduce avoidable distractions and allow planned movement. Praise the behaviour you want to reinforce, such as beginning promptly or waiting for a turn.
Track when difficulties occur, what happened beforehand and what helped. Bring examples from home, school or work to a clinician. A care plan may combine environmental adjustments, skills coaching, psychological support and medication when appropriate.
When to seek care
Arrange an assessment when attention, impulsivity, activity or emotional reactions repeatedly disrupt learning, work, relationships, driving or basic routines. Earlier review is also sensible if behaviour changes suddenly, follows a head injury or appears alongside major sleep or mood changes.
Seek urgent help if the person may harm themselves or someone else, cannot be kept safe, becomes severely confused, or has extreme agitation with loss of contact with reality. Use local emergency services for immediate danger.
