The short answer
Sustained effort to organise, filter distractions, regulate emotions and recover from intense focus can drain energy. Sleep disruption, medication patterns, burnout and physical illness may add to the problem.
Clues behind the tiredness
Mental fatigue may rise after demanding concentration or masking restlessness. A medicine wearing off can feel like an energy crash, while taking a stimulant too late may disturb sleep. Irregular meals, dehydration and alcohol can further reduce alertness.
Fatigue can also accompany anaemia, recent COVID-19, coeliac disease, chronic kidney disease, depression, thyroid problems or a sleep disorder. Chronic fatigue syndrome has a particular symptom pattern and is not diagnosed from tiredness alone. The timing and accompanying symptoms guide the assessment.
Practical ways to respond
Track sleep, meals, fluids, activity, medication timing and energy peaks for several days. Break demanding tasks into shorter sessions, put recovery time after high-focus work and keep wake time consistent. Gentle activity may help, but pushing through marked post-activity worsening can be counterproductive.
Review persistent exhaustion with a GP rather than increasing caffeine or changing ADHD medicine yourself. The appointment may cover sleep quality, snoring, mood, diet, bleeding, infections and substance use. Examination or tests depend on those findings.
When tiredness needs urgent attention
Seek urgent care if exhaustion comes with chest pain, severe breathlessness, fainting, new confusion, one-sided weakness, black or bloody stools, or inability to stay awake. Contact a clinician promptly for worsening fatigue with fever, unexplained weight change, marked thirst, swelling, persistent vomiting or thoughts of self-harm.
