Adenomatous goitre: what it means and when to seek care

It often develops as thyroid tissue grows unevenly over time. The gland may still make a normal amount of hormone, or it may become overactive or underactive. The word adenomatous describes the nodular growth pattern; it does not by itself mean cancer.

The thyroid sits at the front of the neck. An adenomatous goitre can appear as a general swelling or as several distinct lumps. A clinician may examine how it moves when you swallow and ask about neck pressure, voice change, swallowing, breathing, heat or cold intolerance, tremor, bowel changes and changes in energy or weight.

Assessment commonly includes thyroid blood tests and an ultrasound. A scan, or sampling of a particular nodule with a fine needle, may be considered when the examination or ultrasound shows a feature that needs closer evaluation.

Arrange a routine clinical assessment for a new or changing thyroid swelling. Bring a list of medicines and supplements, especially anything containing iodine or thyroid hormone, and mention previous neck radiation, thyroid treatment and close family thyroid history. Do not start iodine supplements or alter prescribed thyroid medicine without clinical advice.

Management depends on gland size, hormone function, ultrasound findings and symptoms. Monitoring may suit a stable goitre without troublesome features. Medicine, radioactive iodine or surgery may be discussed when hormone excess, progressive enlargement, pressure effects or a concerning nodule is present.

Seek urgent care if neck swelling makes breathing difficult, causes noisy breathing, or prevents you from swallowing liquids or saliva. Prompt assessment is also appropriate for rapid enlargement, a new persistent hoarse voice, a hard fixed lump, or enlarged neck glands.

Contact a clinician soon if you develop a racing or irregular heartbeat, marked tremor, unusual drowsiness, pronounced weakness, or worsening heat or cold intolerance, because these can accompany altered thyroid function.

Ask whether the whole gland or particular nodules are enlarged, whether your thyroid hormone results are normal, and whether ultrasound monitoring or needle sampling is appropriate. It is also useful to clarify which change in size, swallowing, voice or breathing should bring you back sooner.
