Adalimumab for ankylosing spondylitis: what patients should know

It is a biologic medicine that blocks tumour necrosis factor, a signalling protein involved in inflammation. The aim is to lessen spinal pain and stiffness, improve movement, and help protect day-to-day function. Response varies, so your specialist will review symptoms, examination findings, and relevant tests rather than assuming the medicine is working.

A rheumatology specialist may consider adalimumab when ankylosing spondylitis remains active despite an appropriate plan that can include anti-inflammatory medicine and movement-based care. It is given by injection under the skin. Your care team should teach you how to store, prepare, and inject it, and where used injection supplies should go.

Because adalimumab changes part of the immune response, infections may be easier to develop or harder to clear. Before starting, clinicians commonly check for tuberculosis and hepatitis, review current infections and vaccination needs, and consider other conditions or medicines that affect immunity. Live vaccines may not be suitable during treatment, so arrange vaccines with the prescribing team.

Tell the prescriber about fever, an infection being treated, recent close contact with tuberculosis, planned surgery, pregnancy plans, breastfeeding, vaccinations, and every medicine or supplement you use. Do not combine biologic medicines or change the injection schedule unless the specialist directs you.

Contact your treating team promptly if you develop fever, persistent cough, shortness of breath, painful urination, a spreading skin infection, shingles-like blisters, unusual bruising, marked weakness, or an infection that is not settling. Ask for advice before the next injection when you are acutely unwell or taking antibiotics.

Seek emergency care for difficulty breathing, swelling of the face or throat, faintness, or a rapidly worsening allergic reaction. New severe weakness, loss of vision, confusion, or other sudden neurological symptoms also need urgent assessment. Mild redness or discomfort at the injection site can occur, but worsening swelling, warmth, discharge, or fever warrants clinical advice.

Ask what improvement would show that adalimumab is helping, when your response will be reviewed, and what the alternative plan is if benefit is limited. Confirm which screening and monitoring tests you need, which vaccines are appropriate, what to do during an infection or around surgery, and whom to contact about a missed dose or injection problem.
