The short answer

Symptoms may be intense at first and then ease as the inflammation settles and the brain adapts to altered balance signals. A clinician should assess a first or severe episode because other ear, neurological and circulation problems can cause similar symptoms.

What is happening in the inner ear

The labyrinth contains organs for balance and hearing. Inflammation can make the balance signals from one ear disagree with information from the eyes and the other ear, producing vertigo and unsteadiness. Hearing reduction or ringing can occur when the hearing part of the labyrinth is involved. Vestibular neuritis mainly affects balance and generally does not cause hearing loss.

The illness may follow a viral respiratory infection, although the precise virus is often not identified. Examination of the ears, eye movements, balance, hearing and nervous system helps separate it from other causes.

Managing symptoms and recovery

During severe spinning, rest somewhere safe, move your head slowly and sip fluids regularly. Avoid driving, ladders and operating machinery while dizzy. A clinician may offer short-term medicine for nausea or vertigo; prolonged use can sometimes slow the brain's balance compensation.

As symptoms settle, gentle walking and ordinary head movement usually support recovery. Persistent imbalance may benefit from vestibular rehabilitation exercises chosen by a trained professional. Arrange a hearing check if hearing feels reduced or uneven.

Seek emergency care for new facial droop, arm or leg weakness, trouble speaking, double vision, fainting, a sudden severe headache, inability to stand, chest pain or loss of consciousness. These features require prompt assessment for causes beyond the inner ear.
