Acute prostatitis: symptoms and next steps

It can cause fever, pelvic or lower-back pain, painful urination, urgency, a weak stream, and difficulty emptying the bladder. Antibiotics are commonly required after a clinician evaluates severity and obtains suitable samples.

The prostate sits below the bladder and surrounds part of the urine channel. Infection can make the gland swollen and tender, obstruct urine flow, and trigger pain between the scrotum and anus, in the penis, or across the lower abdomen. Acute prostatitis commonly presents with urinary symptoms alongside feverishness or chills.

Assessment may include a urine test, urine culture, blood tests, and a gentle examination. Forceful prostate massage is avoided during acute infection because it can push bacteria into the bloodstream. Imaging may be considered if an abscess or urinary blockage is suspected.

Seek a same-day medical assessment rather than waiting for urinary symptoms to settle. Tell the clinician about fever, vomiting, urine flow, recent urinary procedures or catheters, medicine allergies, and previous infections. If possible, provide a urine sample before the first antibiotic dose, but do not postpone urgent treatment to collect one.

While awaiting care, drink according to thirst unless you have been told to limit fluids, rest, and use only pain relief that is safe with your health conditions. Do not take leftover antibiotics, because the choice must cover likely urinary bacteria and may be adjusted when culture results return.

Go to urgent or emergency care if you cannot pass urine, feel faint or confused, have severe weakness, develop shaking chills, keep vomiting, or notice rapidly worsening pain. Inability to urinate may require bladder drainage planned in a way that avoids further prostate irritation.

Breathing difficulty, mottled skin, extreme drowsiness, or a marked decline in alertness can signal infection affecting the whole body. Call emergency services rather than driving yourself if these symptoms occur.

Ask when culture results will be available, whether the antibiotic may change, and how urine retention would be managed. Clarify the follow-up plan and whether persistent fever, worsening flow, or recurring symptoms should prompt imaging or specialist review.
