What should you know about acute heart failure symptoms?

The pattern differs between people. Fluid in the lungs may cause breathlessness at rest, wheezing, or a cough, while fluid in the body may cause swollen ankles or abdominal fullness. Reduced circulation can produce cool skin, weakness, dizziness, or confusion.

Breathing that becomes worse when lying flat or wakes someone from sleep can reflect lung congestion. A sudden fall in exercise tolerance may appear as needing to stop during a familiar walk or becoming breathless while dressing. Palpitations can accompany a rhythm disturbance that has strained the heart.

Symptoms alone cannot confirm the diagnosis because lung disease, infection, kidney problems, and other conditions can look similar. Clinicians combine the history and examination with oxygen measurement, an electrocardiogram, blood tests, and imaging when appropriate.

If breathing has newly worsened, stop exertion and sit upright while arranging urgent assessment. Note when the change began, what activity brings it on, whether lying down worsens it, and whether swelling, chest discomfort, fever, or a racing heartbeat is present.

Do not dismiss symptoms simply because ankle swelling is absent. Acute heart failure may mainly affect the lungs. Likewise, swelling alone has many possible explanations and needs clinical context rather than self-diagnosis.

Call 998 in the UAE when breathlessness is severe, the person cannot speak comfortably, lips look blue or grey, fainting or new confusion occurs, chest pressure develops, or the cough produces pink frothy fluid. Keep the person upright if tolerated and do not let them drive.

Arrange urgent clinical review for worsening breathlessness with usual tasks, repeated waking short of breath, increasing ankle or abdominal swelling, persistent palpitations, or much less urine. A rapid change is more concerning than a stable long-standing symptom.
