Acute gout treatment: controlling a painful flare

Treatment may include an anti-inflammatory medicine, colchicine, or a corticosteroid. The suitable choice depends on when the flare began, which joints are involved, kidney function, stomach or bleeding risks, other illnesses, and medicines already being taken. Resting and cooling the affected joint can support comfort, but they do not replace medical assessment when the diagnosis is uncertain.

A flare develops when urate crystals trigger intense inflammation inside a joint. Gout can present with sudden joint pain, warmth, redness, swelling, and marked tenderness, often making even light contact uncomfortable. Similar symptoms can occur with a joint infection, so a first episode, an unusual pattern, or fever deserves prompt clinical review.

Medicine used to lower urate over the long term has a different purpose from medicine used to calm a current flare. If you already take urate-lowering treatment, do not stop or change it without guidance; abrupt changes can complicate control.

Contact a clinician early because flare medicines tend to be more useful when started promptly. Keep pressure off the joint, raise it if swollen, and apply a wrapped cool pack for short periods. Drink normally unless a clinician has restricted your fluids. Avoid taking several anti-inflammatory products together, and check before using them if you have kidney disease, a history of stomach bleeding, take blood thinners, or may be pregnant.

After the flare settles, discuss urate control, recurring attacks, medicine adherence, and health conditions that influence treatment. Long-term prevention is planned separately rather than judged by how the joint feels on one day.

Seek urgent assessment for a hot, swollen joint with fever, chills, rapidly spreading redness, severe illness, or inability to bear weight, particularly if the joint was recently operated on or injected. These features can accompany infection and may require joint fluid testing and rapid treatment. Get immediate help for facial swelling, breathing difficulty, black stools, vomiting blood, or a severe rash after medicine.

Ask what supports the diagnosis, whether joint fluid needs testing, which flare medicine fits your kidney and stomach health, and what side effects should make you stop it. You can also ask whether urate-lowering treatment is appropriate, how it will be monitored, and what plan to follow at the first sign of another flare.
