Acute hip bursitis: symptoms, care and warning signs

Acute hip bursitis is irritation of a fluid-filled cushioning sac, usually causing tenderness and pain around the outer hip.

The short answer
Pain may be worse when lying on the affected side, climbing stairs, walking farther than usual or rising from a low seat. Similar pain can arise from nearby tendons or the lower back, so an examination may be needed when the diagnosis is unclear.
What is happening at the hip
Bursae reduce friction where tissues glide over bone. At the outer hip, sudden overload, a direct knock, repeated pressure or altered movement can irritate a bursa and the surrounding tissues. The area may be sharply tender when pressed, while discomfort can spread down the outside of the thigh.
The phrase hip bursitis is often used for outer-hip pain even when nearby gluteal tendons contribute. Pain felt deep in the groin, marked loss of hip movement or symptoms coming from the back may suggest a different source. Visible joint swelling is not a defining feature of a deeply placed hip bursa.
How to settle the flare
Temporarily reduce activities that clearly provoke the pain rather than stopping all movement. Avoid lying directly on the sore side; when lying on the other side, a pillow between the knees may reduce compression. A wrapped cold pack can be tried for comfort, with a cloth barrier to protect the skin.
As pain allows, gradually restore walking and hip-strengthening work. A physiotherapist can adjust exercises and identify movement or workload factors. Anti-inflammatory medication is not suitable for everyone, so check with a pharmacist or doctor if you have stomach, kidney, heart or bleeding concerns or take other medicines. A cortisone injection may be discussed when symptoms remain limiting, but it is not automatically required.
When the hip needs assessment
Seek urgent medical assessment for a hot, red or rapidly enlarging swelling, fever, chills or feeling acutely unwell, because infection needs to be excluded. Immediate care is also appropriate after a significant injury if you cannot bear weight, the leg looks deformed or pain is severe.
Book a routine review if the pain persists, keeps returning, disrupts sleep despite avoiding pressure, or makes your walking progressively more difficult. New numbness, weakness, groin pain or pronounced stiffness should also be described because these features can redirect the examination.
Useful questions at your visit
Ask whether the pain appears to come from the bursa, gluteal tendons, hip joint or back, and which findings support that view. Clarify which activities to modify, how to progress strengthening, and what the benefits and downsides of medication or injection would be for you. If infection or another fluid-filled swelling is suspected, ask whether imaging, blood tests or aspiration would change management.

Is it helpful to rest hip bursitis completely?
Complete rest can reduce conditioning without addressing the trigger. Relative rest means easing clearly painful loads while retaining comfortable movement, then rebuilding activity gradually.
Can hip bursitis cause pain down the leg?
Outer-hip pain can spread along the outside of the thigh. Pain extending below the knee with numbness, tingling or weakness may indicate nerve irritation and deserves assessment.
Is joint aspiration a routine treatment for outer-hip bursitis?
No. Aspiration has specific diagnostic or treatment uses and is not routinely needed for every episode of outer-hip pain. A clinician may consider it when the location, swelling or concern about infection makes sampling useful.
