What does acute ankylosing spondylitis mean?
A flare may affect the lower back, buttocks, hips or other joints and can disrupt sleep and movement. It does not mean the entire disease began suddenly, and a new severe symptom should not automatically be attributed to the condition.
Recognising a possible flare
People with ankylosing spondylitis often experience inflammatory back pain and stiffness that is worse after rest and may loosen with gentle movement. Fatigue, reduced range of motion, tender tendon attachment sites or swelling in a peripheral joint can accompany a flare.
Pain can also come from muscle strain, infection, fracture or nerve compression. A change from your established pattern deserves attention, particularly after trauma or when neurological symptoms appear.
What to do during worsening symptoms
Follow the flare instructions agreed with your rheumatology team. Balance short periods of rest with comfortable movement, change position regularly, and continue only the stretches or exercises that do not cause sharp or radiating pain. Heat may ease stiffness if skin sensation is normal.
Use anti-inflammatory or other prescribed medicine according to its directions rather than changing the dose yourself. Record symptom location, morning stiffness, sleep disturbance, eye symptoms and how daily function has changed; this helps the clinical team adjust care.
Urgent symptoms during a flare
Get urgent help for new leg weakness, numbness around the groin, loss of bladder or bowel control, or severe spinal pain after a fall. A painful red eye with light sensitivity or blurred vision needs same-day assessment. Fever, breathing difficulty or a hot swollen joint also warrants prompt medical review.
Preparing for a flare review
Ask whether the current pattern is consistent with inflammatory activity, how medicines should be handled, which exercise level is suitable now, and whether eye, nerve or joint findings require another specialist or imaging.

Should I stay in bed during an ankylosing spondylitis flare?
Extended bed rest can add to stiffness. Unless a clinician has told you otherwise, alternate necessary rest with gentle movement and frequent position changes within a comfortable range.
Can I continue physiotherapy when symptoms worsen?
The programme may need temporary modification rather than complete abandonment. Tell the physiotherapist about the change and stop any movement causing sharp, radiating or progressively increasing pain.
Is every episode of back pain a flare?
No. Mechanical injury and other medical causes can coexist with ankylosing spondylitis. New pain after trauma, pain with fever or neurological changes requires separate assessment.
