Actinic keratosis and actinic cheilitis

It commonly affects the lower lip and may cause ongoing dryness, scale, pale or red areas, blurring of the lip border, fissures or tenderness. Persistent lip change deserves clinical assessment rather than repeated treatment as ordinary chapping.

The lip has a thin, exposed surface with less protective pigment than nearby skin. Accumulated ultraviolet damage can affect a broad section rather than one discrete spot, which is why a clinician examines the full lip, its border and the inside of the mouth.

Actinic cheilitis is precancerous, not a statement that invasive cancer is already present. Squamous cell carcinoma of the lip may present with a persistent ulcer, firm thickening, bleeding or a focal growth; a biopsy is used when examination cannot safely distinguish the changes.

Arrange a dermatologist assessment and mention how long the lip has looked or felt different. Stop peeling scale and avoid unprescribed acids, as irritation can mask the pattern. A biopsy may target the thickest, sorest or most irregular area.

Treatment can be directed at a focal lesion or a wider damaged field. Depending on the findings, options may include a prescribed topical medicine, cryotherapy or photodynamic therapy. Use a broad-spectrum lip balm with sun protection, reapply after eating or drinking, and add a wide-brimmed hat when outdoors.

Seek prompt review for an ulcer that persists, a new firm or raised area, repeated bleeding, increasing pain, rapid change or a patch that does not heal. These signs require examination for squamous cell carcinoma regardless of how much sun exposure you remember.

Urgent medical attention is appropriate for major lip swelling with breathing or swallowing difficulty, which points to an acute problem rather than typical actinic cheilitis.

Ask whether the change is diffuse or focal, which area would be biopsied if needed, and how treatment might affect eating, drinking and lip care while healing. Discuss the expected reaction, when persistent crusting becomes concerning and how the rest of the face and sun-exposed skin will be examined.
