Actemra for rheumatoid arthritis: treatment and safety

Its active ingredient, tocilizumab, blocks the receptor for interleukin-6, an immune signal involved in inflammation. It is a disease-modifying treatment rather than a simple pain reliever. A rheumatologist may prescribe it alone or alongside another arthritis medicine, depending on previous treatment, health conditions and tolerance.

Rheumatoid arthritis commonly presents with painful, swollen and stiff joints, often including the hands and fingers. Actemra aims to control the underlying immune-driven inflammation, which can ease symptoms and help protect function. It may be given as an injection under the skin or as an infusion into a vein.

Response is assessed over time using the pattern of symptoms, joint examination, ability to manage daily tasks and blood results. Because blocking interleukin-6 can lower inflammatory markers and soften signs such as fever, the clinical team considers how you feel as well as laboratory values.

Before the first dose, share any history of tuberculosis exposure, hepatitis, recurring infection, liver disease, low blood counts, bowel inflammation or previous medicine reactions. Review vaccinations and all prescription, non-prescription and herbal products with the care team. Live vaccines are generally avoided during treatment.

Attend requested tests for blood cell counts, liver function and cholesterol. Learn the correct storage and injection technique if using a prefilled device, and use a sharps container. Keep a record of doses and symptoms. If a dose is missed, follow the clinic's instructions rather than doubling the next dose.

Report fever, chills, ongoing cough, breathlessness, mouth sores, painful blisters, unusual tiredness or urinary symptoms promptly. Tell the team before a dose if you are unwell or have started antibiotics. Do not pause or restart Actemra independently unless emergency clinicians advise you to do so.

Emergency assessment is needed for difficulty breathing, facial or throat swelling, collapse or a rapidly spreading severe rash. Obtain urgent advice for strong, persistent abdominal pain with fever or altered bowel habits. Unusual bruising, yellowing of the skin or eyes, or dark urine should be reported without delay.
