Swelling varies between people and injuries, may be subtle, or may become noticeable later. Its absence cannot confirm that the ligament is intact, just as swelling by itself cannot diagnose a tear.

Clues beyond visible swelling

An ACL injury may follow a planted-foot twist, abrupt deceleration or awkward landing. Features that raise concern include a pop at the time, loss of confidence in the knee, giving way during turns, reduced motion or difficulty continuing activity. Other knee injuries can create similar features, so the pattern is not diagnostic on its own.

A clinician compares both knees, assesses motion and uses specific stability tests. MRI may help define the ACL and associated structures when the examination is unclear or when imaging would alter the treatment plan.

Managing a suspicious knee injury

Pause sport and avoid pivots, jumps and sudden direction changes if the knee feels unstable. Reduce weight-bearing when walking is painful, and use a cloth-wrapped cold pack for comfort. Do not repeatedly test whether the knee will buckle.

Schedule a clinical review when giving way continues, motion stays restricted or pain prevents ordinary activity. The lack of swelling should not delay review when function remains clearly abnormal.
