Acid reflux and Zyrtec: what to know

Zyrtec is a brand of cetirizine, an antihistamine that blocks histamine. It does not reduce stomach acid or stop stomach contents moving into the oesophagus, so it should not replace a reflux medicine recommended by a clinician or pharmacist.

Acid reflux occurs when stomach contents travel back into the oesophagus. It can cause burning behind the breastbone, a sour taste, regurgitation, throat irritation or cough. Cetirizine is used for allergy symptoms such as sneezing, a runny or itchy nose, watery eyes and hives.

A cough can have more than one trigger. Reflux may present with cough, while nasal allergy can cause mucus to drain into the throat and provoke coughing. Cetirizine may ease the allergy component, but any reflux can remain and may need separate management.

Check the product label to confirm whether you have plain cetirizine or a combination product. Take it only as directed on the label or by your clinician, and do not double a missed dose. Cetirizine can cause sleepiness, tiredness or dry mouth; avoid driving if it makes you drowsy.

For reflux, notice whether large meals, late eating, lying down soon after food or particular foods worsen symptoms. Smaller meals and remaining upright after eating may help. A pharmacist can advise whether an antacid or acid-reducing medicine is suitable after checking your other medicines, pregnancy status, kidney or liver conditions, and symptom pattern.

Seek emergency assessment for new or severe chest pain, pressure or tightness, especially with shortness of breath, sweating, faintness, or pain spreading to the arm or jaw. Do not assume these symptoms are reflux.

Arrange prompt medical review if swallowing is difficult or painful, food feels stuck, you vomit blood, pass black stools, have persistent vomiting, or lose weight without trying. Book a routine review when reflux keeps returning, disturbs sleep, or continues despite appropriate pharmacy treatment.

Stop cetirizine and obtain urgent help for trouble breathing or swallowing, swelling of the mouth or tongue, collapse, or another severe allergic reaction.

Ask whether your cough or throat symptoms fit reflux, allergy, both, or another cause; whether your exact Zyrtec product is appropriate; and which reflux treatment fits your health history. Bring the packages or a list of all medicines, supplements and herbal products so possible interactions and overlapping side effects can be checked.
