Why discomfort may appear before eating

The stomach continues producing digestive fluid between meals. An empty stomach does not itself establish reflux, but long gaps may make burning, nausea or upper-abdominal discomfort more noticeable. Coffee, smoking, certain medicines or a large meal after fasting may add irritation for some people.

True reflux more often involves burning behind the breastbone, sour fluid rising, or symptoms affected by bending and lying down. Gnawing pain centred in the upper abdomen can have other causes, so the location and pattern matter.

Changes to try

Avoid very long gaps if they reliably precede symptoms. Try modest, balanced meals at regular times and eat slowly rather than responding to intense hunger with a large portion. Choose a simple snack if appropriate for your dietary needs, and avoid lying flat immediately after eating.

Keep notes on where the discomfort sits, whether eating relieves or worsens it, and any sour taste, nausea or medicine use. A pharmacist can help you choose occasional relief and identify products, including some pain relievers, that may irritate the digestive tract.

When the pattern needs checking

See a clinician if episodes persist despite meal changes, regularly disturb sleep, or are accompanied by painful or difficult swallowing, repeated vomiting, unexplained weight loss, loss of appetite, black stools, or blood in vomit. Persistent upper-abdominal pain should not automatically be labelled reflux.

Get urgent help for severe chest tightness, breathing difficulty, faintness, heavy sweating, or pain spreading to the jaw, arm or back. These symptoms need rapid assessment rather than a snack or reflux remedy.

Ask whether the symptom location fits reflux, whether your medicines could irritate the stomach, and whether testing is appropriate. Explain how fasting, eating, posture and bowel changes affect the discomfort.
