Spitting up blood with acid reflux: what to do

Blood may come from the airways, stomach or oesophagus, mouth, gums, or nose. The safest response depends on how much there is, whether it is being coughed or vomited, and whether breathing difficulty, chest pain, faintness, or black stools are present.

Reflux can inflame the oesophagus, but visible blood is not a routine reflux symptom. Blood brought up with nausea or vomiting may arise from the upper digestive tract. Blood mixed with mucus after coughing may come from the respiratory tract, while blood noticed after brushing, nosebleeding, or throat irritation may begin in the mouth or nose.

Colour and texture can offer clues but cannot establish the source at home. Fresh blood may look bright red; altered blood from the stomach may appear dark or grainy. Swallowed blood from a nosebleed can also later be vomited, which is why the events immediately before it matter.

If active bleeding is occurring, sit upright, avoid food and drink until urgent-care staff advise you, and do not drive yourself if you feel weak or light-headed. Seek prompt medical advice even if the bleeding has stopped, particularly when the source is uncertain or the episode recurs.

Tell the clinician whether the blood followed coughing, retching, vomiting, a nosebleed, dental bleeding, or swallowing difficulty. Mention abdominal or chest discomfort, stool colour, recent injury, and every medicine you take, especially medicines that affect clotting or irritate the stomach. Do not stop a prescribed medicine unless a clinician instructs you to do so.

Call emergency services for ongoing or substantial bleeding, breathing difficulty, choking, severe chest or abdominal pain, fainting, confusion, marked weakness, cold clammy skin, or vomit that looks dark and grainy. Black, sticky stools alongside blood from the mouth are also an emergency warning sign.

A small streak still warrants timely assessment if it has no clear source, happens again, or accompanies fever, persistent cough, painful swallowing, unexplained weight loss, or worsening reflux symptoms. Urgency comes from the bleeding and accompanying signs, not from whether reflux is common.

Ask which source is most consistent with the episode, whether examination or testing is needed, and how your medicines affect bleeding risk. Clarify what recurrence should prompt emergency care and how any reflux treatment should be handled while the cause is investigated.
