Acid reflux and shortness of breath

Reflux occurs when stomach contents move upward into the oesophagus. Irritation reaching the throat may trigger coughing, throat clearing or a tight sensation that makes breathing feel less comfortable. Symptoms that follow meals, bending or lying down, together with heartburn or a sour taste, can fit this pattern.

Breathlessness has many other possible causes. Asthma commonly presents with wheeze, cough and shortness of breath, while anaemia may present with tiredness and breathlessness on exertion. Chest or breathing symptoms should not be attributed to reflux without considering these alternatives.

Notice whether symptoms relate to particular foods, large meals, bending or bedtime. Smaller meals, remaining upright after eating and raising the head end of the bed may reduce upward flow. Avoid tobacco smoke, and discuss any frequent need for antacids with a clinician or pharmacist.

Keep a brief record of heartburn, cough, wheeze, activity and breathlessness. This can help a clinician decide whether reflux treatment is reasonable or whether breathing tests, an examination or other checks are needed.

Call emergency services for severe or rapidly worsening breathlessness, blue or grey lips, fainting, confusion, swelling of the lips or tongue, or breathing difficulty with chest pressure or pain. Anaphylaxis can present with sudden breathing trouble and swelling or a widespread rash; this requires emergency treatment.

Arrange a timely medical review if breathlessness is new, keeps returning, wakes you, limits usual activity, or occurs with wheeze, fever, palpitations, leg swelling or a persistent cough.

Ask whether the timing points to reflux, which other causes fit your breathing pattern, whether any medicines could worsen either symptom, and what change should trigger urgent care. Mention swallowing difficulty, voice change, black stools or unplanned weight loss if present.
