Understanding acid reflux pain

Short answer

Acid reflux can cause burning chest or upper-abdominal pain, often after meals or when lying down, but chest pain needs careful assessment.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Common location
Behind the breastbone or in the upper abdomen
Possible triggers
Meals, bending, and lying flat
Emergency concern
Severe or unfamiliar chest pain with systemic symptoms

The short answer

The discomfort may rise behind the breastbone and occur with a sour taste, belching, or fluid returning to the throat. Pain alone cannot confirm reflux because heart, lung, muscle, gallbladder, and other digestive problems can produce overlapping sensations.

Recognising the pattern

Reflux pain often follows a large or rich meal, bending, or lying flat. It may improve after an antacid, yet improvement does not rule out another cause. Note whether swallowing, exertion, breathing, movement, or pressing the chest changes the pain.

Upper-abdominal discomfort can accompany reflux, but severe localised pain, persistent tenderness, fever, or repeated vomiting points away from straightforward heartburn and merits medical review.

Practical relief and review

For familiar mild symptoms, eat smaller portions, remain upright after food, loosen tight clothing around the waist, and limit items that reliably trigger your reflux. Avoid repeatedly testing foods that have already caused marked discomfort.

A pharmacist can help select an antacid or alginate and check it against your health conditions and medicines. If pain occurs regularly, wakes you, or returns after treatment stops, a clinician can assess the diagnosis and discuss longer-term acid suppression or further investigation.

Chest pain safety

Call emergency services for crushing, heavy, severe, or unfamiliar chest pain, particularly when it comes with breathlessness, sweating, nausea, faintness, or pain spreading into the arm, jaw, shoulder, or back. Do not drive yourself if you feel seriously unwell.

Prompt assessment is also needed for difficulty swallowing, food sticking, vomiting blood, black stools, repeated vomiting, unexplained weight loss, or pain that steadily worsens. These features require evaluation even when reflux has been diagnosed before.

Preparing for a consultation

Describe the quality, location, triggers, and duration of the pain, plus any regurgitation, swallowing problems, cough, breathlessness, or exertional symptoms. Ask whether your current medicines irritate the stomach or oesophagus and what treatment review would be sensible.

Questions people ask

Where is acid reflux pain usually felt?

It is often felt as burning behind the breastbone or in the upper abdomen and may travel upward toward the throat. Location alone cannot identify the cause.

How can I tell reflux pain from heart pain?

There is no dependable home test. Meal-related burning with regurgitation supports reflux, while exertional pressure or pain with sweating or breathlessness is concerning, but symptoms can overlap.

Why does reflux hurt more when I lie down?

Gravity no longer helps keep stomach contents down, so acid can more readily reach and irritate the oesophagus.
