Why reflux and nausea can occur together

Hormonal changes in pregnancy can relax the muscular valve between the oesophagus and stomach. As the pregnancy progresses, pressure from the growing uterus may also make stomach contents travel upward more easily. Reflux commonly presents with burning behind the breastbone, a sour or bitter taste, burping, or food and fluid rising into the throat. Nausea may accompany these symptoms.

Nausea that occurs without burning or regurgitation may be part of pregnancy-related nausea rather than reflux. Symptoms that follow a large, rich or spicy meal, worsen when bending over, or begin soon after lying flat are more suggestive of reflux. Keeping a brief note of meals, timing and symptoms can reveal a useful pattern.

Ways to ease symptoms

Try smaller meals, eat slowly, and stop before feeling overly full. Remain upright after eating and avoid meals close to bedtime. Notice whether fatty, spicy or acidic foods, chocolate, caffeine, or fizzy drinks trigger symptoms for you; triggers vary, so broad food restriction is rarely necessary. Sip fluids regularly if nausea makes drinking difficult.

At night, raising the head and upper chest rather than adding pillows only under the head may reduce backflow. Loose clothing around the waist can also help. Speak with a pharmacist, midwife or doctor before taking an antacid, alginate or acid-reducing medicine during pregnancy. They can check whether the product is suitable, how it interacts with prenatal supplements or other medicines, and when it should be taken.

When to seek medical care

Arrange medical advice if symptoms are frequent, disrupt sleep, make eating difficult, do not improve with simple measures, or require repeated over-the-counter treatment. Also seek assessment for pain or difficulty when swallowing, ongoing vomiting, unexplained weight loss, or persistent abdominal pain.

Seek urgent care for vomiting blood, material resembling coffee grounds, black stools, severe chest pain, fainting, marked breathlessness, severe or worsening abdominal pain, or an inability to keep fluids down with very little urine or pronounced dizziness. During pregnancy, vaginal bleeding, leaking fluid, severe headache with visual changes, or reduced fetal movement after movements have become established also need prompt maternity assessment.
