Acid reflux in the first trimester

Short answer
First-trimester acid reflux can result from hormonal changes, even before the uterus becomes noticeably larger.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Early mechanism
- Hormones can relax the lower oesophageal muscle and slow digestion
- Symptom clue
- Burning or sour fluid after meals fits reflux more closely than nausea alone
- Practical record
- Note meal timing, vomiting, burning, and whether fluids stay down
In brief
It may cause burning behind the breastbone, an acidic taste, burping, or stomach contents rising towards the throat. Early-pregnancy nausea and vomiting can overlap with these sensations, so describing the timing and character of symptoms can help a clinician distinguish them.
What changes early in pregnancy
Hormonal changes can loosen the valve-like muscle at the bottom of the oesophagus and slow digestion. This makes upward movement of acidic stomach contents easier. Reflux in the first trimester does not confirm pregnancy, because the same symptoms have many non-pregnancy causes.
Nausea is usually felt as an urge to vomit, whereas reflux more often produces burning, sour fluid, or discomfort after eating or lying down. A person can have both. Repeated retching may also irritate the oesophagus and intensify burning.
Managing reflux alongside nausea
Eat small amounts at a pace you can tolerate and avoid letting a large meal stretch the stomach. Sip fluids between meals if drinking with food worsens fullness. Stay upright after eating, and keep a simple note of foods, meal timing, nausea, and burning to identify personal patterns.
Do not start bicarbonate remedies, herbal preparations, or medicines simply because they are sold without a prescription. Ask a pharmacist, midwife, or doctor which antacid or reflux treatment is appropriate in early pregnancy and how to separate it from folic acid, iron, or other medicines.
Signs that need assessment
Contact your maternity or primary-care team if reflux repeatedly interferes with eating or sleeping, if vomiting prevents you from keeping fluids down, or if you feel dizzy or pass much less urine. Trouble swallowing, painful swallowing, blood in vomit, black stools, or unexplained weight loss also require medical review.
Get urgent help for severe chest pressure, breathing difficulty, collapse, or pain spreading beyond the chest. In early pregnancy, severe or one-sided lower abdominal pain, shoulder-tip pain, fainting, or vaginal bleeding needs urgent pregnancy assessment rather than being attributed to indigestion.
What to discuss at an appointment
Tell your clinician whether symptoms began before or after a positive pregnancy test, how often you vomit, and whether fluids stay down. Ask which treatment fits your current medicines, how long to try it, and what change in symptoms should prompt another review.

Questions people ask
Can reflux start before I look pregnant?
Yes. Hormonal effects can alter digestion early, before pressure from an enlarging uterus becomes a major factor.
Is first-trimester heartburn proof that I am pregnant?
No. Reflux is not a pregnancy test and can occur for several reasons. Use an appropriate pregnancy test and seek clinical advice if the result or your dates are uncertain.
What if nausea makes the usual reflux advice difficult?
Choose small portions and fluids you can tolerate, and seek advice early if intake is falling. A clinician can address nausea and reflux together while considering your pregnancy and other medicines.
