Treatment for an acid reflux cough

Cough can take longer to settle than heartburn because an irritated throat and cough reflex may remain sensitive. Treatment should therefore have a clear plan for use, review and reassessment rather than continuing indefinitely without benefit.

A clinician considers when the cough occurs, whether meals or lying down trigger it, and whether heartburn, sour regurgitation or hoarseness accompanies it. Examination and a review of current medicines may reveal another explanation, such as asthma, nasal drainage or a medicine-related cough.

Testing is not necessary for every person. It may be considered when symptoms are unusual, swallowing is affected, initial treatment does not help, or the diagnosis remains uncertain. The findings determine whether reflux treatment should continue or the search should shift to another cause.

Meal adjustments can reduce reflux reaching the oesophagus: eat smaller portions, avoid lying down soon after food and limit only the foods or drinks that repeatedly trigger your symptoms. Raising the bed head may help a cough linked to lying flat. Stopping smoking reduces throat irritation as well as other causes of cough.

Medicines may neutralise acid, form a barrier above stomach contents or reduce acid production. The correct choice and timing depend on symptom frequency, other conditions and medicines already taken. Follow the label or prescription, and arrange review if the planned course brings little change; do not keep increasing the dose yourself.

Call for urgent help for severe chest pain, sudden or marked difficulty breathing, fainting, blue or grey lips, coughing up blood, vomiting blood or passing black sticky stools. These features can signal problems that require prompt investigation.

Seek a routine but timely review if cough continues for several weeks, affects sleep or daily activity, or occurs with wheeze, fever, weight loss, persistent vomiting, painful swallowing or food sticking. A changing cough also warrants reassessment even if reflux was diagnosed previously.

Ask what evidence in your symptom history supports reflux, which alternative causes remain possible, and when improvement should be reviewed. Clarify the exact medicine timing, likely side effects, interactions and the next step if coughing does not ease.
