Acid reflux and sore throat

A reflux-related throat problem may occur with sour taste, regurgitation, hoarseness, frequent throat clearing or a dry cough. Heartburn may be absent, so the timing and combination of symptoms are useful when a clinician assesses the cause.

The oesophagus has defences against occasional reflux, but the voice box and throat are more sensitive to irritation. Symptoms may be noticeable on waking, after bending, or after eating a large meal before bed.

Fever, swollen neck glands, marked pain on swallowing, runny nose or close contact with someone who is unwell may point toward an infection instead. Dry air, smoke, voice strain, allergies and some medicines can also make the throat sore. A symptom pattern can guide assessment but does not establish the cause by itself.

Choose smaller evening meals, stay upright after eating and leave time between dinner and lying down. Identify repeat triggers such as very rich meals, without adopting an unnecessarily restrictive diet. If nighttime symptoms recur, raising the head end of the bed may help more than stacking pillows.

Drink enough water, avoid smoking and give a strained voice time to rest. A pharmacist can suggest a short-term reflux treatment or simple throat relief after checking your medicines, pregnancy status and medical conditions. Antibiotics do not treat reflux and are not useful for most viral sore throats.

Seek prompt medical advice if swallowing is painful or difficult, food feels stuck, hoarseness persists, reflux keeps returning, or there is unexplained weight loss, a neck lump, blood in saliva or vomit, or black stools. Severe dehydration or inability to swallow fluids also needs urgent attention.

Call emergency services for breathing difficulty, swelling of the tongue or throat, inability to swallow saliva, or severe chest pain with breathlessness, sweating, faintness or spreading pain. Do not wait for antacid treatment to work when these warning signs are present.

Explain when the soreness began, whether it is worse in the morning, and whether you have heartburn, cough, fever, nasal symptoms or voice changes. Ask if throat examination, a medicine review or a monitored reflux-treatment trial would help clarify the cause.
