Acid reflux and heart failure: telling symptoms apart

Reflux often causes burning behind the breastbone, a sour taste, or discomfort after eating or lying down. Heart failure commonly presents with breathlessness during activity or when lying flat, reduced exercise tolerance, fatigue, and swelling around the ankles or legs. Cough can occur with either condition. Symptoms alone may not reliably identify the cause, and new chest discomfort should not be assumed to be indigestion, particularly in someone with heart or circulation problems.

Continue heart failure medicines exactly as prescribed unless your treating clinician changes them. Do not alter a diuretic, fluid limit, or salt plan to manage reflux on your own. Smaller meals and staying upright after eating may reduce reflux. Check any antacid or acid-suppressing medicine with a pharmacist or clinician because some products contain sodium or can affect how other medicines are absorbed. Keep a record of chest symptoms, breathlessness, swelling, daily weight if advised, meal timing, and medicine changes.

Call emergency services for severe or persistent chest pressure, marked breathlessness at rest, fainting, blue or grey lips, confusion, or pain spreading to the arm, jaw, back, or shoulder. Contact your heart failure team promptly if breathing or swelling worsens, you suddenly need more pillows to breathe comfortably, or your weight changes beyond the limit in your personal care plan. Difficulty swallowing, vomiting blood, black stools, or unexplained weight loss also require medical assessment.

Take an up-to-date list of prescription medicines, supplements, and remedies. Ask how to distinguish your usual reflux from a change in heart symptoms, which reflux treatment fits your cardiac medicines, and whom to contact if breathing, swelling, or weight changes. Confirm whether you have individual advice about fluids, salt, monitoring, and sleeping position. Report palpitations, dizziness, reduced urination, loss of appetite, or new nighttime coughing.
