Telling the patterns apart

Reflux tends to cause burning behind the breastbone, sour-tasting fluid, regurgitation, throat irritation, or cough, especially after eating or lying down. Lactose intolerance more often causes bloating, wind, cramping, loose stools, or diarrhoea after lactose-containing food because lactose is not fully digested.

Milk itself is not a universal reflux trigger. A rich or high-fat dairy meal may delay stomach emptying and aggravate reflux in a person who is susceptible, while someone with lactose intolerance may tolerate low-lactose products. Milk-protein allergy is different again and can involve hives, swelling, wheeze, vomiting, or other immune symptoms.

How to identify a food-related trigger

Keep a food and symptom diary that records the type of dairy food, portion, other foods in the meal, timing, and whether symptoms are upper digestive or bowel-related. Change one feature at a time. Comparing a lactose-free version with the usual product may help separate lactose symptoms from a reaction to meal size or fat content.

Avoid removing all dairy for long periods without dietary advice, particularly for children, during pregnancy, or when nutrition is already limited. A clinician or dietitian can help maintain protein, calcium, and vitamin intake and decide whether breath testing or another assessment is appropriate.

When symptoms need medical attention

Call emergency services for swelling of the lips, tongue, or throat, difficulty breathing, collapse, or rapidly spreading hives after food. These features suggest a serious allergic reaction rather than ordinary intolerance or reflux.

Request medical review for difficulty swallowing, food sticking, blood in vomit, black stools, ongoing vomiting or diarrhoea, dehydration, severe abdominal pain, unexplained weight loss, or symptoms that persist despite careful dietary changes.

Ask whether your symptom pattern suggests reflux, lactose intolerance, milk-protein allergy, or another digestive condition; how to perform a safe food trial; which nutrients need replacing; and whether testing would change management. Does dairy cause acid reflux?
