This attachment-site inflammation is called enthesitis. It can cause tenderness, stiffness, warmth or swelling behind the heel, and it may occur alongside back stiffness or symptoms at other tendon and joint sites.

Inflammatory pain versus tendon overload

Achilles pain in someone with ankylosing spondylitis may reflect enthesitis, ordinary tendon overload, or a combination of both. Pain that is prominent after rest and accompanied by broader morning stiffness may point toward active inflammation, while a clear increase in walking, running or jumping load may support a mechanical contribution. Neither pattern confirms the cause on its own.
