Acetone testing in urine

Urine strips mainly reflect ketones that have accumulated since the bladder last emptied. The result must be considered alongside symptoms, food intake, diabetes status, medicines, and sometimes a blood ketone measurement.

Ketones can occur after fasting, prolonged vomiting, a very low-carbohydrate diet, or strenuous activity. They can also build up when diabetes leaves the body without enough effective insulin.

A positive urine result alone does not identify the cause or show the current blood ketone level. Dehydration and the timing of urination can affect what the strip shows, and an older urine result may lag behind a changing illness.

Follow the strip instructions exactly, use a fresh sample and unexpired strips, and compare the colour within the stated reading window. Record the result together with symptoms and, if you monitor diabetes, the glucose reading.

If you have diabetes, follow your personal sick-day plan and use blood ketone testing when your care team has recommended it. Do not stop insulin because you are eating less. Contact your diabetes team for ketones that persist, increase, or occur with high glucose or illness.

Seek emergency care for ketones with repeated vomiting, abdominal pain, deep or rapid breathing, marked thirst, frequent urination, fruity-smelling breath, severe weakness, drowsiness, or confusion. These can accompany diabetic ketoacidosis, which needs hospital treatment.

Pregnancy, diabetes, or use of an SGLT2 diabetes medicine lowers the threshold for urgent clinical advice because dangerous ketoacidosis can sometimes develop without a strikingly high glucose reading.
