Location does not reveal the underlying contributor by itself. A clinician combines the body map with the speed of onset, medicines, family pattern and relevant metabolic or hormonal symptoms.

Look in good light and note whether the patch is velvety, sharply bordered, scaly, itchy or tender. Check comparable folds on both sides, but avoid rubbing repeatedly to test whether the colour comes off. A photograph can help document spread.

Keep folds dry without over-washing, minimise friction and use a simple moisturiser if the surface feels rough. Do not apply a strong exfoliant to the armpit, groin or beneath the breasts without guidance, because product tolerance varies by site.

Arrange an earlier assessment if the change appears abruptly across many areas, progresses quickly, or affects the lips or inside of the mouth. New widespread thickening together with unexplained weight change or feeling generally unwell also merits prompt evaluation.

A sore fold with cracks, discharge, heat or odour may have superimposed irritation or infection. Seek direct care for those features rather than assuming every symptom belongs to acanthosis nigricans.
