Acanthosis nigricans on the face

It may be noticeable around the forehead, temples, cheeks, or mouth and can occur alongside similar changes on the neck or in other folds. Facial pigmentation has several possible causes, so colour alone cannot establish that acanthosis nigricans is present.

A clinician looks at texture, distribution, timing, other affected sites, medical history, and current medicines. Flat pigmentation from sun exposure, irritation, melasma, or a previous rash can resemble the colour change but does not have the same velvety thickening.

When acanthosis nigricans is confirmed, the assessment may explore insulin resistance, diabetes, hormonal symptoms, and medicine-related triggers. The condition can present with dark patches; a dark facial patch on its own does not show which diagnosis is responsible.

Choose a non-abrasive cleanser, a simple moisturiser, and daytime sun protection suited to your skin. Stop scrubs, bleach, and homemade acid mixtures, which can cause inflammation and leave additional pigmentation. Do not apply a treatment intended for thick body skin close to the eyes or lips unless a clinician confirms it is suitable.

Managing an identified underlying condition is central to care. For remaining texture or colour, a dermatologist may consider a carefully introduced topical retinoid, a chemical peel, or an energy-based treatment after examining skin tone and sensitivity. These options can irritate or alter pigmentation and require individual selection.

Arrange an appointment when facial darkening is new, progressive, difficult to identify, or accompanied by velvety patches elsewhere. Tell the clinician about thirst, frequent urination, menstrual changes, weight changes, and any recent medicine changes so the examination is interpreted in context.

Prompt assessment is appropriate if pigmentation develops abruptly and spreads, involves the mouth, or appears with significant itching or unexplained general symptoms. Swelling, blistering, raw skin, or eye involvement after applying a product needs timely care for a possible irritant or allergic reaction.

Ask what features distinguish this from melasma or post-inflammatory pigmentation, whether other body areas should be examined, and whether metabolic or hormonal testing fits your history. If treatment is offered, ask where to apply it, how to recognise irritation, and which products in your current routine should be paused.
