Acanthosis Nigricans and Diabetes

Insulin resistance may present with velvety, thickened, darker skin in body folds. The same appearance can occur with other metabolic or hormonal factors, certain medicines, inherited patterns, or less common medical conditions, so an individual assessment matters.

When body tissues respond less effectively to insulin, the body may produce more of it to keep glucose regulated. High circulating insulin can stimulate growth signals in skin cells, contributing to the characteristic thickened and velvety surface. Common sites include the back and sides of the neck, armpits, and groin.

Diabetes can exist without acanthosis nigricans, and acanthosis nigricans can exist without diabetes. Skin appearance cannot reveal a glucose result or distinguish insulin resistance from diabetes. Blood testing and the broader clinical history make that distinction.

Arrange a primary-care review if the condition is newly recognised or metabolic status is unknown. The clinician can consider blood glucose testing alongside weight history, blood pressure, medicines, family history, and symptoms. Treat folds gently with mild cleansing, moisturiser if dry, and measures that reduce friction. The colour is not caused by poor hygiene. Once underlying factors are addressed, a dermatologist may consider a topical retinoid or a supervised procedure for persistent texture or pigmentation, with care to avoid irritation.

Seek timely assessment if dark velvety patches appear quickly, spread widely, involve the lips or mouth, or occur with unplanned weight loss or feeling generally unwell. Those features need examination for causes beyond routine metabolic risk.

Prompt diabetes assessment is also warranted for marked thirst, frequent urination, blurred vision, unusual fatigue, or unexplained weight change. Painful cracks, warmth, discharge, or odour in a fold can indicate a secondary skin problem that needs separate treatment.
