What the term absence seizure means

The word ‘absence’ refers to the brief gap in awareness, not to the person being physically absent. The event reflects a temporary change in the brain’s electrical activity. It may be subtle enough to look like daydreaming, especially when there is no collapse or forceful limb movement.

Speech or movement may stop suddenly. The eyes can remain open, with rapid blinking or small repetitive movements around the mouth or fingers. Calls or a light touch do not bring an immediate response, and the person then returns abruptly to what they were doing.

This description differs from ordinary distraction, which tends to develop less abruptly and can usually be interrupted. It also differs from a convulsive seizure with falling, stiffening, and prominent jerking. The appearance alone is not enough to diagnose the seizure type because fainting, sleepiness, medicines, migraine, and low blood glucose can also affect awareness.

Write down what happened just before the pause, whether the person answered, which movements occurred, and how they behaved afterwards. Time future events rather than estimating their length. If recording is safe and does not delay assistance, a video may help the clinician see the pattern.

Arrange an appointment after a first suspected event or repeated unexplained lapses. The assessment may include questions about sleep, medicines, illness, alcohol exposure, and family history, followed by an electroencephalogram or selected blood tests. Maintain supervision around swimming, bathing, heights, traffic, flames, and powered equipment until the risk has been discussed.

Seek emergency help when altered awareness lasts five minutes, another seizure starts before the person recovers, breathing is abnormal, the skin becomes blue or grey, injury is serious, or weakness persists. Move hazards away, support the head if the person falls, turn them onto their side when practical, and keep the mouth clear.

Urgent evaluation is also warranted when seizure-like activity accompanies fever, neck stiffness, a severe headache, a non-fading rash, or marked difficulty waking, since meningitis can present this way. Sweating, shakiness, confusion, or collapse may accompany hypoglycaemia and should be handled promptly, particularly in someone using glucose-lowering medication.

Ask what alternative explanations fit the observed spell, how an electroencephalogram would inform the diagnosis, and whether current medicines need review. Confirm the precautions appropriate for work, school, driving, sport, and water, as well as who should receive a seizure action plan.
