NPO instructions for abdominal ultrasound

Some examinations require no food beforehand, while others also require water and a full bladder. The imaging centre's instructions take priority because preparation varies with the organs being viewed and the appointment time.

Food can cause the gallbladder to contract, and swallowed air or bowel gas can block sound waves. Fasting can therefore help the sonographer obtain clearer views of upper abdominal organs. For the bladder and nearby pelvic structures, urine inside a full bladder creates a useful viewing window.

NPO does not automatically mean withholding prescribed medicines. Medicine instructions need individual guidance, especially when tablets must be taken with food or when fasting could alter diabetes management.

Read the booking message closely for the permitted time of the last meal, whether plain water is allowed, and whether you should avoid smoking or chewing gum. Contact the centre before the appointment if the wording is unclear, you are pregnant, you have diabetes, or fasting may be medically difficult.

If you ate or drank against the instructions, tell the imaging team rather than hiding it or cancelling on your own. They can decide whether useful images are still possible, whether the examination can be adjusted, or whether it should be rescheduled.

Fasting instructions should never delay urgent medical attention. Seek prompt assessment for severe or escalating abdominal pain, repeated vomiting, fainting, confusion, fever with abdominal pain, yellowing of the skin or eyes, blood in vomit or stool, or an inability to keep fluids down. Tell the treating team when you last ate, drank, and took medicine.

Confirm what NPO means for your particular scan, whether water is permitted, and which regular medicines may be taken. If you use insulin or other glucose-lowering treatment, request a specific plan for meals, medicine, and monitoring. Also ask whether a full bladder is required and what to do if that becomes uncomfortable.
