Short answer

Exercise-related abdominal pain often comes from a side stitch, muscle strain, meal timing, or digestive upset.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Do not ignore
Pain with collapse, chest pressure, severe breathlessness, or a rigid abdomen

The short answer

The location, activity, recent food and fluid intake, and associated symptoms offer useful clues. Pain that settles after slowing down is less concerning than intense discomfort that continues at rest or occurs with collapse, chest symptoms, or repeated vomiting.

Patterns that can explain the pain

A side stitch is a sharp or pulling pain, commonly beneath the ribs, that appears during running or other repetitive movement. A strained abdominal muscle tends to hurt with twisting, coughing, sitting up, or pressing the affected spot. Neither pattern should cause fainting or a rigid abdomen.

Eating a large meal shortly before activity can contribute to cramping, reflux, nausea, or an urgent bowel movement. Dehydration, heat, abrupt increases in training load, and intense exertion may add to discomfort. Pain unrelated to movement may instead reflect a digestive, urinary, reproductive, or other abdominal condition.

How to respond and reduce recurrence

When pain starts, reduce your pace or stop, breathe slowly, and note exactly where it hurts. Gentle pressure over a side stitch and relaxed breathing may help. Do not push through worsening pain, dizziness, unusual breathlessness, or weakness.

For future sessions, allow time after a substantial meal, sip fluids according to conditions and thirst, warm up gradually, and build intensity in manageable increments. Track which movements, foods, drinks, and training conditions precede symptoms. Persistent or recurring pain deserves an appointment even if it fades after exercise.

When exercise pain needs prompt care

Call emergency services for abdominal pain with fainting, confusion, chest pressure, severe breathing difficulty, bluish lips, or collapse. Seek urgent assessment for sudden severe pain, a hard or swollen abdomen, blood in vomit or stool, persistent vomiting, high fever, or major injury.

Stop exercising and seek same-day advice if pain remains significant after rest, repeatedly returns at a similar effort level, or accompanies dark urine, inability to keep fluids down, painful urination, or new vaginal bleeding. A clinician may examine the abdomen and decide whether heart, lung, digestive, muscle, urinary, or pelvic evaluation is needed.

What to discuss at an appointment

Ask whether the pain pattern suggests a stitch, muscle injury, digestive problem, or another condition. Bring details about the exercise, intensity, meal timing, hydration, medicines, bowel or urinary changes, and how quickly symptoms resolve. Ask what activity is safe while the cause is being assessed.

Questions people ask

Should I continue exercising through a side stitch?

Reduce the intensity and stop if needed. Resume cautiously only after the pain has settled and no warning symptoms are present.

Can eating before exercise cause abdominal pain?

A substantial meal close to vigorous activity can contribute to cramping, reflux, or nausea. Recording meal timing can help identify a personal pattern.
