Abdominal pain when sexually active

Sexual activity may trigger pain during penetration, but pain at other times can still be related to the reproductive, urinary, or digestive system.

A missed or unusual period makes pregnancy testing relevant, including when contraception was used. Pelvic inflammatory disease can present with lower abdominal pain, pain during sex, unusual discharge, bleeding between periods, or fever. Endometriosis can present with pelvic pain that follows the menstrual cycle, deep pain during sex, or painful bowel movements.

Pain at the vaginal entrance may reflect dryness, irritation, a vulval condition, or pelvic-floor tension. Cramping, bloating, constipation, urinary burning, and one-sided pain each shift attention toward different possibilities, but no single symptom confirms the cause.

Take a home pregnancy test if pregnancy is possible and the period is late, lighter than expected, or otherwise unusual. Stop sexual activity that provokes pain. If exposure to a sexually transmitted infection is possible, arrange confidential testing and use barrier protection or avoid sex until results and any treatment are complete.

Keep brief notes on the pain site, menstrual timing, contraception, discharge, bleeding, urinary changes, bowel changes, and whether penetration triggers it. A clinician can then decide whether a pelvic examination, pregnancy test, infection swabs, urine testing, or ultrasound is appropriate.

Seek urgent medical care for severe or rapidly worsening abdominal pain, fainting, dizziness, shoulder-tip pain, heavy vaginal bleeding, persistent vomiting, or pain with a positive pregnancy test. These warning signs require assessment for an acute pregnancy-related or pelvic problem.

Fever, feeling very unwell, foul-smelling discharge, or increasing pelvic tenderness also warrants prompt attention because infection may need treatment. Arrange a routine appointment for milder pain that keeps returning even if it settles between episodes.

Ask whether pregnancy and infection have been excluded, what the pain pattern suggests, which tests are useful now, and how to manage intimacy safely while waiting for a diagnosis or response to treatment.
