Abdominal pain during sex: what to know

The location, timing, and associated symptoms help narrow the possibilities. Recurrent pain deserves assessment rather than being endured or worked around indefinitely.

Pain at the vaginal opening may relate to dryness, irritation, vulvodynia, a skin condition, or pelvic-floor muscles tightening. Pain felt deeper in the pelvis or lower abdomen can occur when penetration puts pressure on sensitive structures.

Endometriosis and ovarian endometrioma can present with deep pain during sex, often alongside painful periods. Pelvic inflammatory disease can present with lower abdominal pain and pain during sex, sometimes with unusual discharge, bleeding, or fever. Symptoms alone cannot establish which condition is present.

Stop if intercourse hurts. Until the cause is clearer, try slower activity, more arousal time, lubricant, shallower penetration, or positions that let you control depth. Record whether pain is at the entrance or deep inside, whether it lasts afterwards, and how it relates to periods, discharge, bleeding, urination, or bowel movements.

Arrange a clinician visit if pain recurs, limits intimacy, or comes with other pelvic symptoms. Assessment may include questions about menstruation and contraception, an abdominal or pelvic examination, infection testing, or imaging according to the pattern.

Seek urgent medical assessment for sudden or severe lower abdominal pain, fainting, dizziness, shoulder-tip pain, heavy bleeding, fever, persistent vomiting, or pain with a possible pregnancy. These features can signal bleeding, infection, or another acute pelvic problem.

Prompt care is also sensible for worsening pain with foul-smelling discharge, pain when passing urine, or bleeding after sex. Urgency comes from these warning signs, not from guessing how common a particular diagnosis might be.

Ask which features point toward surface pain, pelvic-floor tension, infection, or a deeper pelvic condition; whether infection testing or ultrasound fits your symptoms; and what pain-control options are appropriate while the cause is investigated.
