Abdominal oedema: causes and warning signs

The area may feel puffy, tight or heavy, and gentle pressure may sometimes leave a temporary indentation. Swelling can be localised to the skin or occur alongside enlargement deeper within the abdomen, so the cause cannot be determined from appearance alone.

Clinicians consider when the swelling began, whether it is one-sided, and whether the legs, face or other areas are also affected. Recent surgery, injury, a new medicine, skin inflammation and problems with fluid circulation may all be relevant. Red, hot and tender skin can accompany cellulitis, while angioedema commonly presents with sudden deeper swelling and may also involve the lips, tongue or throat.

Hives can present with raised, itchy swellings that move or change shape. By contrast, persistent abdominal fullness may come from tissue or organs beneath the skin rather than oedema. Examination helps distinguish these patterns and directs any blood tests, imaging or medication review.

Note the onset, speed of change, discomfort, skin colour and swelling elsewhere. List medicines and recent procedures, and take a photograph if the contour varies during the day. Avoid tight compression around the abdomen and do not start water tablets, antihistamines or leftover antibiotics without guidance; each treats a different mechanism and may obscure useful clues.

A clinician may examine the skin and abdomen, review heart, kidney, liver, lymphatic and allergic features, and choose tests according to the whole pattern. Treatment targets the cause rather than the swelling alone.

Call emergency services for swelling with breathing difficulty, throat or tongue swelling, faintness, confusion, blue lips or severe rapidly developing symptoms. Obtain urgent assessment for intense abdominal pain, a rigid or quickly enlarging abdomen, repeated vomiting, fever, rapidly spreading redness or very tender skin. New swelling after surgery also merits prompt contact with the treating team.

Ask whether the swelling appears to be within the skin, beneath it or deeper in the abdomen; whether any medicine could contribute; and which accompanying signs should trigger urgent care. Clarify how to monitor the area and when reassessment is needed if it persists or spreads.
