Abdominal pain during ovulation

This pattern is sometimes called mittelschmerz. Its position may alternate between cycles, although location and calendar timing cannot rule out pregnancy complications, infection, ovarian problems, or digestive and urinary causes.

The discomfort may be a dull ache, cramp, pressure, or sharper twinge low on either side. Stretching of the developing follicle and irritation from fluid released around ovulation may contribute. A small amount of spotting or clear, slippery cervical mucus may occur around the same phase of the cycle.

Ovarian cysts can present with pelvic pressure, aching, or pain on one side, while endometriosis can present with recurrent pelvic pain and pain linked with periods, sex, bowel movements, or urination. Those presentations overlap with other conditions, so symptoms cannot establish either diagnosis on their own.

If discomfort is mild and you otherwise feel well, rest, gentle movement, hydration, or warmth over the lower abdomen may help. Ask a pharmacist which pain relief is suitable for you, particularly if you could be pregnant, take other medicines, or have stomach, kidney, liver, or bleeding concerns.

Use a cycle diary to note which side hurts, the character of the pain, bleeding or discharge, and any relation to sex, meals, urination, or bowel movements. Repeated entries give a clinician more useful information than an estimated ovulation date alone.

Obtain urgent help for sudden severe pain, collapse or fainting, marked dizziness, shoulder-tip pain, heavy vaginal bleeding, fever, persistent vomiting, breathing difficulty, or a swollen rigid abdomen. Pelvic pain with a positive pregnancy test also needs prompt assessment, especially when it is one-sided or accompanied by bleeding.
