How the exercise relates to diastasis recti

Diastasis recti involves the connective tissue at the abdominal midline, not simply a weak muscle that needs to be pulled inward. A vacuum may help someone sense lower-abdominal engagement, yet appearance during one exercise does not show how well the trunk manages pressure during coughing, carrying, or rising.

Forceful hollowing can also encourage breath-holding or excessive tension. Those patterns may be unhelpful if they produce dizziness, pain, midline doming, downward pelvic pressure, or urine leakage. Treatment should respond to function and symptoms rather than chase a completely flat abdomen.

How to approach it safely

Begin in a supported position and breathe without straining. After a relaxed exhale, gently narrow the lower abdomen while keeping the ribs, jaw, and pelvic floor comfortable; then release and breathe normally. Stop if you feel light-headed, painful, or unable to avoid bearing down.

Do not treat stronger suction or a longer hold as proof of better recovery. If the movement is comfortable, a physiotherapist may integrate it with coordinated breathing, progressive resistance, balance, and task-specific lifting. Modify any exercise that causes uncontrolled ridging or worsens pelvic symptoms.

When professional help is useful

Book an assessment if you are unsure how to breathe during the exercise, cannot control bulging, or develop persistent back pain, pelvic heaviness, leakage, or difficulty with normal activity. Seek urgent care for severe or escalating abdominal pain, vomiting, fainting, or a hard tender bulge that will not reduce. Do not attempt to pull such a bulge inward with a vacuum exercise.

You might ask whether abdominal hollowing suits your breathing and pelvic-floor pattern, how much effort to use, and which functional exercises should accompany it.
