In brief

Many clinicians use the broader term greater trochanteric pain syndrome because irritation can involve the gluteal tendons as well as nearby bursae. Pain is often worse when lying on that side, climbing stairs, walking uphill or standing with weight shifted onto one hip.

Why the outside of the hip hurts

Bursae are small fluid-filled sacs that reduce friction where tissues pass over bone. Around the greater trochanter, repeated compression and tendon overload can sensitise the tissues. A sudden increase in walking or running, prolonged side-lying, altered gait or weak hip control may contribute.

Trochanteric bursitis commonly presents with local outer-hip tenderness and pain, rather than deep groin pain. Examination helps distinguish it from hip-joint problems, referred back pain, nerve irritation and injury. Imaging is not always required, but may be considered when the pattern is unclear or progress stalls.

Reducing irritation while rebuilding capacity

Temporarily reduce activities that sharply increase pain, but keep moving within a tolerable range. Avoid hanging on one hip while standing, crossing the legs tightly, or sleeping directly on the sore side. A pillow between the knees can reduce compression when lying on the other side.

Physiotherapy may use gradual strengthening of the hip and trunk, gait advice and a paced return to activity. Pain relief or anti-inflammatory medicine must fit your health history. A cortisone injection may settle symptoms for some people, while shockwave therapy may be considered in selected persistent cases; rehabilitation still addresses load tolerance.

When hip pain needs assessment

Get urgent care after a fall if you cannot bear weight, the leg appears shortened or rotated, or pain is severe. Fever with a hot, red, swollen hip area, rapidly worsening symptoms, or feeling acutely unwell can point to infection and should be assessed promptly.

Arrange a clinical review for persistent night pain unrelated to position, unexplained weight loss, a new lump, progressive weakness or numbness, or pain that continues despite sensible activity changes. New bladder or bowel trouble with leg symptoms requires emergency evaluation.

Should I completely rest the hip?

Usually, relative rest is more helpful than stopping all movement. Reduce clearly aggravating loads while maintaining comfortable activity and beginning a graded exercise plan.

Is trochanteric bursitis the same as hip arthritis?

No. Trochanteric pain arises from tissues around the outer hip, while hip arthritis affects the joint and more often produces deep groin or front-of-thigh symptoms.

Will an injection cure the problem?

An injection may reduce pain, but it does not automatically restore tendon capacity or movement control. Its place should be considered alongside progressive rehabilitation.
