The main point

A flare often brings sudden severe pain, warmth, redness and swelling in one joint, commonly at the base of the big toe, ankle or knee. Symptoms can resemble an infected joint, so a first or unusual episode deserves prompt assessment.

Why gout develops

Urate forms when the body breaks down purines. If blood urate remains high, needle-shaped crystals can collect in and around joints. The immune response to those crystals produces the intense inflammation of a flare.

Kidney function, some medicines, alcohol, dehydration, body weight and dietary patterns can affect urate levels or trigger attacks. Gout can present with joint pain and joint swelling, but those symptoms also have many other causes; diagnosis depends on the clinical picture and sometimes testing joint fluid.

Managing a flare and future risk

During a flare, rest the joint, raise it if practical and use a wrapped cold pack for comfort. Contact a clinician early because anti-inflammatory treatment tends to be more useful when started promptly and must be chosen around kidney, stomach, heart and medication considerations.

Long-term urate-lowering medicine is different from pain relief: it reduces the material from which crystals form. Take it consistently if prescribed and attend monitoring. Hydration, gradual weight management where relevant, and moderating alcohol may support the plan, but lifestyle measures do not replace indicated medicine.

Signs that need prompt attention

Seek urgent medical assessment for a hot, swollen joint accompanied by fever, chills, marked illness, a wound near the joint or inability to bear weight. Joint infection can look similar to gout and requires rapid treatment.

Arrange follow-up if attacks recur, more joints become involved, firm nodules appear around joints or ears, or prescribed medicine causes a rash. Sudden flank pain, vomiting or blood in the urine may signal a urinary stone and also warrants care.

No. The big-toe joint is a familiar site, but gout can affect ankles, knees, fingers, wrists and other joints.
